A dermatologist prescribes isotretinoin for KF, a 20 year old woman with severe nodular acne that did not respond to oral antibiotics. KF is sexually active. Which requirement is most important before and during therapy?
Isotretinoin is highly teratogenic. The Canadian monograph and pregnancy prevention program require negative pregnancy tests before, during and after treatment and the use of two effective forms of contraception from 1 month before, during, and for 1 month after therapy in people who can become pregnant. Vitamin A supplements add toxicity, and dairy and routine eye exams are not the key requirements. Underestimating isotretinoin's teratogenic risk management.
